The Declaration Page, also known as Dec Page, reflects the key points of your
policy. Typically, a declaration page will contain the following information:
- Your insurance policy number
- Names and address of insured
- Name, address, and contact information of the insurance company
- Policy’s date of effectivity
- Types, limits, and deductibles of coverage/s
- Premium, discounts, and surcharges
- Limits of liability
- Vehicle information (model, license plate number)
- Losses and convictions
- Rating information
The declaration page is normally the policy’s first page or first few pages. Because it contains critical information about you and your coverage, you must review that all information entered is true. Among the most crucial to check are the names of the insured, coverage, and the policy amount.
